Your Patient Visit

If it will be your first visit with Durbin Family Practice: Please arrive 30 minutes prior to your scheduled appointment with the doctor. Bring along your insurance card and a form of identification.  Please also bring any medication and/or supplements bottles you are taking. Please complete the online new patient paperwork and be prepared to provide us with your previous doctor’s name, address and phone number to complete a medical records request so that we may obtain your prior medical information.  Please understand without this your  appointment time can be delayed or may need to be rescheduled

If this will be your follow up visit with Durbin Family Practice:  Please arrive 15 minutes prior to your scheduled appointment with your doctor. Also, bring along any new insurance card or updated personal information with you to your appointment. Please also bring any new medications you may have been prescribed by other physicians. Please understand without this your appointment can be delayed or may need to be rescheduled.
